Sample takers’ Registration in primary care
As a sample taker registered with CervicalCheck you must complete a registration form for each Contracted Doctor (CRD) that you take samples under. If you have a change in work location or address please complete an updated Health Professional registration form and submit it to admin@cervicalcheck.ie.
Important
Please remember you are contractually obliged to complete a CervicalCheck Clinical update every 3 years. There is a suite of clinical updates available on www.nssresources.ie from which you can choose from. Your registration will not be complete until you have completed this clinical update requirement.

You must register under each contract holder in the practice who holds a contract with CervicalCheck. This is required because the contract holder’s MCRN number recorded on the cervical screening form identifies which doctor (Clinically Responsible Doctor) holds clinical responsibility for the sample.
Each contracted doctor that you work for must sign a separate Health Professional registration form.
You can start taking samples once you receive your registration confirmation email from the CervicalCheck programme administration office.
Do not take any cervical screening samples before you have received the confirmation email from the CervicalCheck programme administration office.
Once you begin taking samples, you must record your MCRN/NMBI number and name in the form under sample taker details. Please do not record someone else’s MCRN or NMBI number. Please note this number may contain the letter D/N after your registration number. This is your unique CervicalCheck Registration Number.
